Enrollment Packets

Important Enrollment Information for the 2025–2026 School Year
All finalized registration paperwork and the first month’s tuition are due by July 1, 2025.

If you are a new student enrolling for the 2025–2026 school year, please download the full enrollment packet, which includes the following required forms:

• Child Enrollment and Health Information

• Child’s Medical Statement (Note: This physical form is valid for one full calendar year. If your child has had a physical within the past year, please bring this form to your doctor to complete. Submit the completed form along with updated shot records.)

• Tuition Letter

• Pick-Up List

• Permission to Photograph Form

If you prefer a printed packet, you may pick one up at the front of the church office, Monday through Thursday, 9:00 AM – 3:00 PM.

All completed forms and payment must be received by July 1, 2025 to finalize your child’s enrollment. Please email your forms to deboard.sonshine@gmail.com, mail or drop off at the school.

For your convenience, the school supply list is also attached. Supplies can be brought to the Meet and Greet event in August.
